***NEW ATHLETIC CLEARENCE PROCESS***
DVUSD has partnered with Register My Athlete to make the clearence process more efficient and paperless. All forms and medical clearence will be done through the Register My Athlete website. Each student athlete will need to create an account on Register My Athlete and complete all form on their account to be cleared to participate in athetics. Click the link for Register My Athlete
Parent Instructions/Tutorial for Register My Athlete
Clearance Process Checklist:
1. Create Register my Athlete profile for student athlete and fill out all necessary information including E-Signatue documents.
2. Have student athlete complete a sports physical with a Doctor/Physician. Make sure to have the Physical Examination and Evaluation forms signed by the physician. Upload signed forms to correct link in Register my Athlete.
3. Student athlete needs to complete the AIA Brainbook course and upload their Certificate of Completion to Register my Athlete. (See below for link)
4. Parents or Guardians need to complete the Emergerncy Contact form and upload to the student athletes Register my Athlete'e account.

Tryouts & Practices: Tryouts and practices will be closed to the public. Please come support your child during games and tournaments.
Practices are held Mon-Thur 3:15-5:00pm unless otherwise scheduled by the coach. Please be punctual to pick players up from practice.
AIA Concussion Online Course: In compliance with ARS §15-341 (24) (SB 1521) all student athletes shall complete the Brainbook online concussion education course. This course is to be taken one time by a student athlete.

This course was originally set up for high school athletes, but the Deer Valley Unified School District has asked all middle school students to take the course as well. (when the student takes the course they will need to register as a 9th grader. You should also choose the high school that is nearest to you).
Once this course is completed, the student will not need to take the course again in high school.
